As I was driving down the road yesterday I received an overheating message so I pulled over only to find coolant leaking from the bottom passenger side of the vehicle. Upon further inspection it seems that hose #9 popped off of the radiator, spilling fluid everywhere. After forcing it and its clip back on I filled up the reservoir, observed the temperature while idling to be stable, then headed back home without any issues. The temperature stayed at 90 degrees the entire trip.

My question is, what could cause hose #9 to pop off the radiator? Or could it only be bad installation?

Is this a brand new van? Anyways, you gotta love those new quick connect style hoses. I had to replace number 13 in your diagram due to a crack.

First question is , "have you had service done recently?

The van isn't new, it's a 2016 model. I haven't had a service done recently, no. My last service with regards to the cooling system was in February of 2020. If the hose was reinstalled improperly it's amazing that it stayed on for an entire year.
